Support Worker - Key Responsibilities
Provide person-centred support to individuals, promoting independence, choice, dignity, and wellbeing in line with care plans, risk assessments, and Positive Behaviour Support (PBS) plans.
Key Duties:
- Deliver tailored support that meets individuals' physical, emotional, social, and mental health needs.
- Encourage independence, life skills development, and participation in education, employment, volunteering, and community activities.
- Assist with personal care, mobility, nutrition, continence care, and the use of specialist equipment where required.
- Support health and wellbeing by monitoring needs, attending appointments, promoting healthy lifestyles, and maintaining accurate records.
- Administer and manage medication safely in accordance with policies and training.
- Follow PBS strategies, support individuals during periods of distress, and promote least restrictive practices.
- Safeguard individuals by identifying and reporting concerns, promoting rights, dignity, and protection from harm.
- Maintain a safe, clean, and welcoming living environment, supporting daily household tasks and health and safety standards.
- Complete accurate documentation, including daily notes, incident reports, health records, and financial records.
- Support individuals with budgeting, personal finances, and accessing benefits where appropriate.
- Work collaboratively with colleagues, families, healthcare professionals, and external agencies to achieve positive outcomes.
- Adhere to organisational policies, regulatory requirements, and quality standards while contributing to continuous service improvement.
- Participate in training, supervision, and flexible shift patterns, including evenings, weekends, bank holidays, sleep-ins, and waking nights as required.

Senior Support Worker
We are looking for an experienced and compassionate Senior Support Worker to lead shifts and support the delivery of high-quality, person-centred care. You will provide guidance and mentoring to Support Workers while ensuring individuals receive safe, effective support that promotes independence, wellbeing, and positive outcomes.
Key Responsibilities:
- Lead and support staff during shifts, promoting best practice and person-centred care.
- Deliver and oversee tailored support that promotes independence, dignity, choice, and inclusion.
- Support the development and review of care plans, risk assessments, and outcome-focused support plans.
- Monitor individuals' health and wellbeing, liaising with healthcare professionals as required.
- Ensure safe medication management, accurate record keeping, and compliance with organisational policies.
- Support Positive Behaviour Support (PBS) approaches and guide staff in managing complex behaviours.
- Promote safeguarding, human rights, and the wellbeing of individuals at all times.
- Complete audits, quality checks, and contribute to continuous service improvement and CQC compliance.
- Oversee financial records and support individuals with budgeting and money management where appropriate.
- Maintain a safe, clean, and welcoming environment, ensuring health and safety standards are met.
- Act as the senior person on shift, responding to incidents and supporting operational requirements.
- Deputise for the Team Leader or Manager when required.
What We're Looking For:
- Experience in health and social care, ideally in a senior or supervisory role.
- Strong leadership, communication, and organisational skills.
- Knowledge of safeguarding, medication management, PBS, and person-centred support.
- Commitment to delivering outstanding care and achieving positive outcomes.
- Flexibility to work a range of shifts, including weekends, bank holidays, sleep-ins, waking nights, and participation in the on-call rota where required.
